If you have active Medicare coverage, you may be eligible to enroll in Lark's Medicare Diabetes Prevention Program (MDPP). In order to be eligible, you generally need to meet all of the following criteria:
- You have active Original Medicare Part B coverage. Medicare Advantage plans do not cover MDPP, so Part B specifically is required.
- Your BMI meets the program's threshold. This threshold is lower if you identify as Asian or Asian American, in line with CMS guidance for this population.
- You do not have a history of Type 1 or Type 2 diabetes.
- You do not have End-Stage Renal Disease (ESRD).
- You have a qualifying blood test result (such as an A1C test) from within the past 12 months.
- You are not currently enrolled in an MDPP program through another supplier.
