The one-minute eligibility survey uses CDC criteria to assess your risk for type 2 diabetes. If Lark informs you that no program is appropriate for you, it most likely means one of the following:
- Your survey responses indicate a low risk of type 2 diabetes
- Your responses indicate a disqualifying factor for the programs available through your insurance
What you can do
You can view the CDC’s criteria for the Diabetes Prevention Program at the CDC’s website. If you believe you should qualify and received an unexpected result, contact our support team via the webform and we’ll investigate further.
💡 If you’d like to re-take the survey, use the original enrollment link from your invitation and start the process again.
