If you were invited to join Lark’s Weight Management Program (including the GLP-1 Companion sub-program) but currently have a Lark account enrolled in the Diabetes Prevention Program (DPP), you can transition your existing account — you don’t need to create a new one.
Who can transition?
This transition is only available to members who have been confirmed as eligible for the Weight Management Program by their pharmacy benefit manager (PBM) — the organization that manages prescription drug benefits through your health insurance plan.
If Lark has not yet received eligibility information from your PBM, the transition link will not work and you will not be able to complete the transition at this time. Eligibility is determined by your health plan, not by Lark.
⚠️ If you attempt to enroll using the link below and your account is not yet confirmed as eligible, the enrollment process will not complete. If this happens, contact your health plan’s member services team to confirm your Weight Management Program eligibility status.
How to transition your account
Follow these steps exactly. Do not create a new Lark account — signing in to your existing account is what triggers the transition.
- Open the enrollment link provided in your invitation from your health plan or CarelonRx. Or go directly to: https://enroll.lark.com/next/anthembcbs-wms-survey.
- When prompted to create an account, do not create a new one.
- Select “I already have a Lark account — Sign In.”
- Sign in using the same credentials you use for your current Lark account (email and password, or your Google, Apple, or Sydney sign-on).
- Once signed in, your account will automatically transition to the Weight Management Program.
Why do I need to transition instead of using my DPP enrollment?
The Diabetes Prevention Program and the Weight Management Program are separate Lark programs with different coaching tracks, program requirements, and health plan billing. If you are taking or planning to take a GLP-1 medication, you must be enrolled in the Weight Management Program (specifically the GLP-1 Companion sub-program) to fulfill your program requirements — DPP enrollment does not satisfy this requirement.
What happens to my DPP account after the transition?
After you transition, your account moves to the Weight Management Program. Here’s what to expect:
- Your coaching track will update to reflect the Weight Management Program.
- Your existing conversation history, logged health data, and completed missions are preserved.
- Your DPP program will end — you will no longer receive DPP-specific coaching or be able to continue toward DPP program milestones.
- If you were working toward a DPP Fitbit® incentive, contact our support team via Settings → Help Center to ask about your eligibility status before transitioning.
ℹ️ If you have any trouble during the transition — such as an error on the enrollment page or not being recognized as eligible — contact our support team via Settings → Help Center. Include your name, date of birth, and the email address on your Lark account.
