Lark’s programs are offered at no cost through participating health insurance plans and employers. Not all insurers cover Lark.
How to check your eligibility
- If you received an email, text, or mailer from Lark — use the QR code or link included to check your eligibility. This is the most accurate way to confirm coverage.
- If you heard about Lark another way — visit lark.com and use the eligibility check tool to see if your plan is covered.
If your insurance isn’t accepted
If you receive an error during enrollment saying your insurance wasn’t found or isn’t covered, it likely means your plan is not currently a participating provider.
You can encourage your insurer to add Lark as a covered benefit by contacting their member support line (the number on your insurance card) and expressing your interest.
If you were interested in the Diabetes Prevention Program specifically, you may be able to find another CDC-certified DPP through the CDC’s website.
