If you're enrolled in Lark's Diabetes Prevention Program (DPP) through Anthem, your Anthem health plan is billed directly — up to four times over one calendar year — as you enroll in and progress through the program. These charges cover things like enrollment, the devices Lark sends you, and program milestones. Lark never bills you directly, and you're never responsible for these costs.

When Does Lark Submit Claims to Anthem?
Claims are submitted based on your engagement and time in the program, not on a fixed monthly schedule. Claims are more frequent early in the program, and the first three are typically submitted within your first three months:
- Claim 1 — submitted when you create your account, install the Lark app, and complete your first coaching conversation.
- Claim 2 — submitted between weeks 2 and 4, based on your engagement with the Lark coach (coaching conversations, meal logging, weight logging, and physical activity).
- Claim 3 — submitted between weeks 4 and 9, based on continued engagement with the Lark coach.
- Claim 4 — submitted when you reach 5 percent weight loss, the CDC's standard for program completion. The timing of this claim depends entirely on your own progress.
Will I Get an Explanation of Benefits (EOB) From Anthem?
Yes. When Anthem processes one of these claims, you may receive an Explanation of Benefits (EOB) describing the cost. An EOB is not a bill — it's a summary from Anthem showing how a claim was handled. Some EOBs list an amount as "patient responsibility," but this does not apply to Lark's program: you will not be asked to pay it.
What Happens if Anthem Denies a Claim?
Nothing changes for you. Even if Anthem denies a claim for coverage, Lark will not bill you and you will not owe anything. This applies to all four claims submitted during the program.
How Long Will Anthem Continue to Be Billed?
Lark remains available to you at no additional cost for as long as you maintain participating Anthem coverage. Once you complete the program — including reaching the 5 percent weight loss milestone for Claim 4 — no further claims are submitted on your behalf.
ℹ️ An Explanation of Benefits describes how your insurer handled a claim. It is not an invoice, and Lark will never ask you to pay the difference.
