Once you have successfully earned your Fitbit® by actively participating in the first ten weeks* of Lark’s Diabetes Prevention Program, Lark will automatically distribute your Fitbit redemption code via email. The process of confirming your activity and sending your redemption code may take a few weeks after you successfully meet the criteria.
If you have not received your code after this period and believe that you qualify, please look for your redemption code in your text messages and the junk or spam folders of the email you provided to Lark. If you are still unable to locate your redemption code, please let us know by reaching out to our support team using our webform. We will be happy to validate whether you successfully earned the reward and provide you with a copy of your redemption code if you’ve successfully met the criteria to earn the Fitbit.
*Eligibility requirements for the Lark Diabetes Prevention Program include qualifying as prediabetic according to a survey designed by the Centers for Disease Control, and maintaining adequate insurance coverage via participating health insurance providers and plans. You must meet participation requirements in the program to qualify for a scale provided at no cost to you and earn a free Fitbit® tracker. Lark requires engagement with the program for a minimum of ten weeks after enrollment to be eligible to earn a Fitbit. Fitbit trackers cannot be earned prior to that time. Device selection and the ability to earn a free Fitbit® tracker vary by health plan. Not all health plans provide the ability to earn a free Fitbit® tracker. Eligibility determinations are made by Lark at its sole discretion.
