The one minute survey prior to the insurance check uses the US Center for Disease Control (CDC) criteria to determine your risk level of developing Type II Diabetes. The survey asks for information about your height and weight, age, ethnicity, medical history, and other factors to determine your risk level for developing Type II diabetes.
You may find that Lark informs you that no Lark program is appropriate based on the information you provided. This means that you have a low risk of developing Type II diabetes or have another disqualifying factor for participation in Lark's programs.
Lark follow's the CDC guidance for determining who is at risk for Type II. You can view the criteria for participation in a Diabetes Prevention Program at the CDC's website, linked here:
If no Lark program is appropriate but believe you should be qualified to participate, please contact our support team by filling out this contact form, and our Support team will be able to investigate further.
